Please give your full name, your birth date and how we may get back in touch with you, besides an e-mail address such as a phone number.

Grabbing Our Attention
Please make certain that the topic category (prescription refill, appointment scheduling etc.) is on the subject line so that we know how to triage your e-mail.

Privacy Issues
Only the staff at SRMS will view the e-mail. Please see our privacy policy for additional information. For established patients, E-mail (both to and from) will become part of your medical record.

Do Not Discuss Topics
We do not suggest that you discuss issues regarding HIV and mental health via e-mail.

Turnaround Times
Please feel free to contact us via any of the above methods. Please give the practice 24-48 hours to respond to non-emergent issues. Emergencies should be taken care of by phone.
